Job Description:
A company is looking for a Telephonic RN Nurse Care Manager.
Key Responsibilities
Coordinates care services with the pod leader to ensure tailored
access to comprehensive services for patients
Develops and manages personalized care plans, addressing care gaps
and ensuring access to resources
Conducts thorough nursing assessments via telephone and provides
education to patients and caregivers on health management
Required Qualifications
Active Registered Nurse License
2+ years of care management experience in community, health plan,
or hospital systems
Strong clinical skills and proactive thinking
Knowledge of Medicare regulations and home care and hospice
standards
Bachelor of Science in nursing or related field preferred
