Job Description:
A company is looking for a Field RN Case Manager in
Illinois.
Key Responsibilities
Manage and coordinate care for patients in a remote setting
Conduct assessments and develop care plans in collaboration with
healthcare teams
Monitor patient progress and adjust care plans as necessary
Required Qualifications, Training, and Education
Current unrestricted RN licensure in Illinois
3-5 years of clinical experience, preferably with Medicare
members
2-3 years of experience in case management or home health care
coordination
Associate's degree with a requirement to obtain a bachelor's degree
within 3-5 years
Certified Case Manager certification is preferred
