    • Travel potential up to 20% We are currently seeking a Physician, Palliative Care to provide geriatric evaluations and management services to SCAN members at the HealthCHEC program. The program has been in operation since 2013 directed by a Geriatrician. This opportunity will allow you to work and collaborate with a Geriatric trained physician and Medical Director, Dr. Dan Osterweil. Dr. Osterweil, MD, FACP, leads the senior-focused HealthCHEC Comprehensive Health Evaluation operations. He completed his geriatrics fellowship at UCLA. His areas of expertise include cognitive and functional assessment, management of dementia and continuous quality improvement. HealthCHEC- Comprehensive Health Evaluation Center is an innovative member centered--program were members of SCAN are invited to complete their Personal Health Assessments.--The Personal Health Assessment includes reviewing and completing a detailed medical history, complete review of tests, screenings and preventative care the member has received, review of current medications and a complete physical exam. Upon completion of the Personal Health Assessment, our--program develops a recommended care plan, discusses it with the member and sends a report to the member's Primary Care Physician.-- Job Summary: --Provide full scope of evaluation and management services including but not limited to diagnosis, treatment, coordination of care, palliative care and health maintenance to patients eighteen (18) years of age and above. Advise the Medical Director on programmatic needs. Support the vision, goals and objectives of the Center and work collaboratively with the HealthCHEC team to achieve them. Your Key Duties and Responsibilities: Assume quality of care for all patients. Examine patients, formulate diagnostic plans, define and order required diagnostic testing. Interpret examination findings and test results, and implement treatment plans. Prescribe and assess effectiveness of pharmaceuticals, other medications, and treatment regimens as appropriate for patient medical conditions. Provide post-acute care to discharged patients both in the Home and select Skilled Nursing Facility settings. Oversee annual chronic care assessments for patients in the home setting. Liaison with other primary care physicians, hospitalists, ED physicians and specialists to coordinate care for post-acute care patients. Collaborate and communicate the management plan for the post-acute patients with the multidisciplinary care team including, but not limited to, case manager, pharmacist, specialists and others. Prescribe medication in accordance with California statute and professional practice guidelines. Arrange referral for patients requiring services not offered at HealthCHEC. Review incoming reports (e.g. lab, x-ray, EKG) sign, date and follow-up in a timely manner. Participate in staff outreach and off-site health care programs in the HealthCHEC Assessment Center service area as assigned by the Medical Director. Comply with the HealthCHEC Assessment Center Infection Control plan, which includes personally utilizing universal precautions. Ensure compliance by support staff. Participate in peer review, quality assurance, provider meetings, and other clinical meetings. Assist the Medical Director and HealthCHEC management in establishing medical policies and quality improvement procedures; evaluating current practices policies and procedures; and updating protocols and principles of practice as requested. Assist in outside medical public relations functions. Comply with all HealthCHEC policies and procedures, as well as pertinent state and federal regulations. Adhere to HealthCHEC productivity and quality standards and meet at least the minimum requirements as determined by the Medical Director. Collaborate with the Nurse Practitioner on new, complex post-acute care patients as assigned by the Medical Director. Collaborate with Social Workers and Nurse Case Manager in providing quality care for patients. Work closely with the HealthCHEC management as it pertains to clinic operations and responsibilities. Other projects and duties, as assigned. Your Qualifications:
      • Medical Doctorate Current California State Medical License in good standing Current D.E.A. Certificate Board Eligible or Board Certified in Internal Medicine. If Board Eligible at the time of hire, must be Board Certified within 3 years of hire date. Geriatric medicine training and or experience, required. Palliative Care training and or experience, required. Bilingual Spanish/English skills are preferred.
        • Travel potential up to 20%- must have CA driver's license with good driving record, proof of insurance, and reliable transportation. What's in it for you?
