Primary Care Lead physician will provide clinical leadership and
oversight of team members on the SCAN Mobile Care Team and provide
direct primary care to empaneled SCAN members. You Will Provide
care in collaboration with a care team to an assigned panel of
members in their place of residence. Collaborate with Integrated
Care Nurse Practitioners and other team members to manage complex
patients by regularly seeing members in tandem and being available
for urgent telephonic or virtual engagement. Serve as a liaison
with other primary care physicians, hospitalists, ED physicians and
specialists to coordinate care for post-acute care patients.
Prescribe medication in accordance with California statute and
professional practice guidelines. Provide clinical leadership
through direct field observations of team members, participating in
interdisciplinary meetings, and collaborating with leadership to
design and implement new workflows intended to enhance care
delivery to members.
- Provide education to Integrated Care clinical team members
related to geriatrics, complex care, and palliative care.
- Provide guidance to clinical team to prioritize member needs
and assist with overall population health management of members
cared for by the SCAN Mobile Care Team.
- Be available and provide support to Integrated Care team
members in the field regarding urgent care needs for members such
as change in medical condition or goals of care discussion. Provide
clinical services and oversight at select skilled nursing
facilities. Support primary NP to manage members in acute rehab
setting. Participate in community stakeholder meetings and address
any issues related to care provided to members as needed.
Participate with Integrated Care Sales team in events to build
member awareness and educate communities. Participate in on-call
program to support team members telephonically after hours and
weekends. Participate in peer review, quality assurance, provider
meetings, and other clinical meetings. Work closely with the SCAN
Mobile Care Team management as it pertains to clinic operations and
responsibilities. Other projects and duties, as assigned. Your
- Doctoral Degree, preferrably with -Geriatrics/Palliative care
- 2+ years of related experience as a physician, preferably with
home-based visits, geriatrics, chronic disease management, and
hospice and/or palliative care.
- Experience with virtual care/telehealth preferred.
- Knowledge of the treatment of complex chronic diseases, such as
dementia, diabetes, heart failure, and COPD.
- Ability to identify team needs and coordinate with other
- Knowledge of HCC and ICD-10 coding and documentation.
- Knowledge of wound care and management.
- Knowledge of CMS guidelines and Medicare Advantage Managed
- Comfortable with initiating goals of care and end of life
- Strong interpersonal skills and change agent mindset needed to
build relationships with healthcare team and patients.
- Cultural competence and sensitivity required, multilingual
- Knowledge of and ability to maintain HIPAA requirements.
- Adaptable and open to change and problem-solving.
- Knowledge of and/or ability to learn basic technological skills
(Word, Excel, PowerPoint, Outlook, EMR) and open to new
- Travel 40+% of the time in the assigned area- Must have a valid
driver's license, automobile insurance and reliable transportation.
