Haven Health, the leading Home Health and Hospice Company in Southern California, is currently hiring a Registered Nurse for Palliative to cover Orange County and Los Angeles area to fill our position within our Hospice division.

Responsibilities:
Evaluate and Admit patients to Palliative Care services based on meeting criteria
Generate a Plan of Care per patient care needs
Collaborate with Interdisciplinary Team on a Plan of Care individualized to patient
Collaborate with Insurance, Managed Groups, Physicians, vendors, and referral sources
Complete all tasks assigned (Admission, Recertification, Routine, Discharge, other)
Manage assigned patient caseload

Requirements:

Qualifications:
Current California Registered Nurse License
Hospice, Home Health, Public Health experience a plus
Experience not required- We will provide training
Excellent written and verbal communication skills
CA licensed and insured driver with reliable personal transportation
