Job Description:
A company is looking for an LPN/LVN Health Coach to optimize
patient medical therapy and serve as a liaison with healthcare
providers.
Key Responsibilities
Work with patients and caregivers to enhance engagement and address
healthcare barriers
Guide patients through medical therapy changes and connect them
with clinicians as needed
Act as the interface between patients and the healthcare system,
contributing to clinical protocol development
Required Qualifications
Associates or equivalent degree required
Active license as a practical nurse or vocational nurse in a
compact state
2+ years of experience in a patient care role
High-speed home internet access
Quiet home office space, free of distractions
