Job Description:

PAY RANGE: $44/hr-$45/hr

REMOTE but candidates to be located in CALIFORNIA only

Help manage patient discharges from hospitals to lower levels of care (like nursing homes or rehab centers).

Must-Have Experience:

CA licensed RN
Experience doing discharge planning (moving patients to SNF, home care, rehab, etc.)
5+ years as a hospital nurse (like in ICU, ER, or Med-Surg)
Medi-Cal rules, MCG guidelines.
