Internal and Family Medicine.
Key Responsibilities
Provide complex care management in primary care settings
Assist patients in overcoming barriers to achieve health-related
goals
Collaborate with a patient-centered team to improve health
outcomes
Required Qualifications, Training, and Education
Bachelor's Degree in Social Work from an accredited institution
Active license by the Maine State Board of Social Work
Experience providing therapeutic services to adult and pediatric
patients with chronic medical issues
Experience in client advocacy and case management
