Physical Therapist - PACE – UHC Staffing Physical Therapist - PACE
Location: San Ysidro, CA Contract Type: Contract Shift Type:
Monday-Friday, 8:00 AM - 5:00 PM (Day) Hours per Week: 40 Candidate

people heal. Roles & Responsibilities Assess PACE participants and
create individualized, goal-driven plans of care Deliver
therapeutic interventions including exercise programs, gait
training, and neuromuscular reeducation Reassess and modify
treatment plans based on participant progress and outcomes
Supervise Physical Therapy Assistants and support staff to ensure
high-quality care Conduct home, hospital, and community visits to
support participants in their environments Educate participants and
caregivers on adaptive devices and self-care strategies Document
evaluations, treatments, and discharge summaries accurately and in
a timely manner Ensure compliance with PACE, CMS, DHCS, and other
regulatory standards Participate actively in interdisciplinary team
meetings to coordinate holistic care Maintain departmental
policies, quality standards, and infection control practices
Qualifications Active California Physical Therapist license
required Bachelor of Physical Therapy (for programs completed
pre-2000), Master of Physical Therapy (post-2000), or Doctorate of
Physical Therapy (post-2015) Valid California driver license,
reliable transportation, and current auto insurance Spanish
strongly preferred to support a diverse patient population
Geriatric experience preferred — 1 year is a plus Supervisory
experience with PTAs and aides preferred New graduates are welcome
and will be considered Education Bachelor's Degree in Physical
Therapy (for programs completed pre-2000) Master's Degree in
Physical Therapy (for programs completed post-2000) Doctorate of
Physical Therapy (for programs completed post-2015) Knowledge &
Skills Strong communication, documentation, and interpersonal
skills Proficient clinical reasoning and patient-centered care
planning Experience with exercise programs, gait training, and
functional activities Knowledge of orthotics, prosthetics training,
and pain management strategies Comfort conducting home and
community-based visits Ability to work effectively in a
multicultural, interdisciplinary team environment Familiarity with
regulatory requirements relevant to PACE, CMS, and DHCS Competent
in electronic medical record documentation and reporting Benefits
