As part of the Homeless Program Care team, support the program
objectives of enhancing older adults', who are housing insecure,
ability to manage their health and well-being while navigating
challenges of housing instability. Role will involve conducting
field assessments to clients "where they are at" in the field
(including but not limited to street, shelters, transitional and
permanent supportive housing, hospitals, parking lots, encampments,
parks, etc.) developing and implementing plans of care, advocating
on behalf of client and their caregivers and communicating
actionable data to client's care team.
Assesses client and field/housing environment to identify physical,
social, and emotional and knowledge barriers to optimal client care
related to health, housing and homeless resources. Identify
pertinent information and communicates to care team.
Facilitates client's health through advocacy, support with
understanding and use of benefits, assistance in scheduling
appointments, development of a support system, and reinforcement of
self-management and organizational tools.
Supports client well-being by using strong observation and
communication skills to coordinate care between client, and
caregiver as well as community resources. Provides ongoing
face-to-face and/or telephonic visits with clients to identify
barriers to accessing care, and supporting the plan of care
developed by Homeless Services team.
Contributes to the development of individualized care plan by using
evidence-based guidelines and clinical knowledge, for client and
provider, by evaluating and conducting phone screenings, field
visits, gathering assessment information, and identifying problems,
goals and interventions.
Assists clients and caregivers in taking an active role in health
management and promotion, through coaching, education, navigation
and referrals to appropriate care and community-based
Comprehensive knowledge of community and public resources serving
the client population. Thorough understanding of how to
access/maintain and actively assists clients to secure
Reviews adherence to care plan, assessing barriers to care, and
documenting information for the care team.
Develops cross-functional relationships with Homeless care team and
Homeless Community care organizations by attending appropriate
meetings, case conferencing, working on-site, and structuring
effective communication mechanisms.
Exhibits strong interpersonal, critical thinking and analytical
skills through positive communication with clients, caregivers, and
care team and community agencies.
Demonstrates excellent organizational, decision-making, critical
thinking and multi-tasking skills as demonstrated by problem
solving and successful outcomes.
Completes timely and accurate documentation in multiple computer
systems to record assessment, observations and corresponding
documentation, including care plans, and progress notes.
Enhances skills and knowledge by participating in educational
offerings and team case conferences and training per department
Utilizes department desktop procedures, workflows, job aids and
training material. Identifies barriers to work processes and brings
to the attention of the supervisor/manager.
Maintains the client's right to privacy and protects SCAN and
Medical Group operations by keeping information confidential.
Adheres to all quality, compliance and regulatory standards to
achieve desired outcomes.
Actively supports the achievement of SCAN's Vision and Goals
Other duties as assigned.
Associate degree or 2+ years college in human services field.
Bachelor's degree preferred. A comparable combination of
education/experience and/or training will be considered equivalent
to the education.
2+ years of experience working with housing insecure
2+ years of experience in community services, preferably a
2+ years working with older and disabled individuals and their
families including caregivers, community-based experience,
Work is primarily in the field (based out of home office).
Must be willing to travel throughout LA County
Working knowledge of medical terminology and abbreviations.
Understanding of confidential information.
Broad understanding of local community homeless resources for older
adults and disabled.
Proficient in MS Office.
BILINGUAL- Must be bilingual in English/Spanish. (Test will be
administered to assess proficiency.)
Occasional travel in the assigned area- Must have a valid driver's
license, automobile insurance and reliable transportation.
