The Job Enhance member satisfaction and support access to quality care by identifying member needs, providing resources to appropriate SCAN and community programs, and proactively assisting members with obtaining necessary healthcare services. Act as a liaison to members, SCAN Health Plan, Provider Organizations and community-based organizations to support members as they learn to self-navigate the healthcare system. You Will * Serve as a single point of contact for referring physicians, members and caregivers to provide resources and assistance with accessing clinical and supportive care services offered.
* Conduct telephonic outreach to members to assist with reported access to care or delay in care concerns to improve coordination of member care and reduce barriers by collaborating with members, caregivers, provider organizations and the SCAN case management team.
* Document information in the care management system during the phone call with the member and/or caregiver regarding identified problem(s), interventions, outcomes, referrals and/or appointments made with/for the member.
* Use pre-defined criteria to refer to care management. Refer to internal or Provider Organization Care Management Programs as needed.
* Collaborate, establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic clinics, internal departments and provider organizations in order to address identified issues.
* Stay informed of changes in the healthcare organization and seek out current information about available member resources. Maintain professional and technical knowledge by attending educational classes.
* Maintain members' right to privacy and protect SCAN operations by keeping information confidential.
* Utilize department desktop procedures, workflows, job aids and training material. Identify barriers to work processes and bring to the attention of the supervisor/manager.
* Adhere to all quality, compliance and regulatory standards to achieve HCS and SCAN outcomes.
* Contribute to team effort by accomplishing related results as needed.
* Actively support the achievement of SCAN's Vision and Goals.
* Other duties as assigned. Your Qualifications Associates Degree or equivalent education/experience required. A comparable combination of education/experience and/or training will be considered equivalent to the education listed. 3 years' experience in health related field or physician office. Strong written and oral communication skills, as well as strong interpersonal, critical thinking, and analytical skills. Demonstrates knowledge of assessment, health and functional problems of older adults and managed care. Ability to prioritize multiple and competing tasks, work independently and in a team environment while working in a fast-paced and dynamic environment. Medical and clinical terminology conversant. BILINGUAL- Strongly Preferred bilingual in English/Spanish. (Test will be administered to assess proficiency.)
